Print a more detailed error message when failing to retrieve a valid
JSON payload from Github.

This will hopefully facilitate troubleshooting of issues such as #56 in
the future.

Revert "Github: Wrong comparison of repository name" ...

This reverts commit a419068 which
introduced a regression issue causing the plugin to fail retrieving the
data from the post_receive hook.

This is due to using the wrong repository attribute to compare with the
repository name in the payload ('name' instead of 'info/hub_reponame').

Github: Wrong comparison of repository name ...

On Github the repository names are not required to be lowercase. The
repository name send in the payload-data by Github could be something
like 'RePo-SiTory_NamE'.

In that case the comparison will fail and no import is possible.

GitHub: Fix MySQL error 1292 when inserting date ...

When MySQL sql_mode is set to strict (STRICT_TRANS_TABLES or
STRICT_ALL_TABLES), the DB rejects the date insertions in ISO-8601
format (yyyy-mm-ddThh:ii-ssZ) as sent by GitHub with error "Incorrect
datetime value".

This commit converts the date to ISO-9075 format as accepted per MySQL
documentation [1]

Fix Github branch name imports ...

When receiving checkin data from Github service hooks, the branch name
should be pulled directly from the ref_name property rather than parsing the
full ref value.  This fixes issues with mantisbt branch names that contain
periods, such as master-1.2.x.

Separate remote import page from user imports ...

Due to complexities of trying to have both user-initiated imports and
remote imports handled in the same page, the import process has been
split into two separate use cases.  pages/import.php is now specifically
for use by the remote import process, while pages/repo_import_latest.php
remains for usage by authenticated MantisBT users.

This will require that all cronjobs or post-commit hooks that triggered
a remote import be updated to point to the new import url.  Framework
version has been bumped accordingly.

Re-licensed the project under the MIT license
