search.php: fix changesets per page ...

Number of changesets per page was not passed on to filter->find() so the
page would always display 25 changesets.

This has not been an issue so far, because $f_perpage is currently
hardcoded to 25.

The new Source_View_Pagination() function removes redundant code from
the search and list pages, and improves it by

- using a closure instead of create_function()
- use MantisBT standard "first/prev/next/last" instead of "<</>>"
- print fewer page links, increasing the gap between them depending on
  total number of changesets (total / 10 rounded to nearest 10)

bugnote_add() API will force VS_PUBLIC state if changeset's author does
not have private_bugnote_threshold.

Use Selection list to pick bugfix notes view state ...

To be consistent with MantisBT core, the bugnote view status is now set
by means of a selection list (i.e. pick public/private) instead of a
checkbox.

Added option to select view state of bug notes ...

Prior to this, the 'Bug fix' notes view state was hardcoded to public.
We are now allowing the administrator to choose whether they should be
public or private by means of a config option.

Improve display of repository 'Extra Info' field ...

Instead of a foreach loop and individual call to var_dump() on each
element of the info array, we now call var_dump() on the array itself,
so that the displayed information is more easy to read.

"Bug Fixed Assign To Committer" option not saved when checkbox unchecked ...

added support for Source/import by name

Using the gpc_get_string() with a default value as @vboctor said.

using $_GET/$_POST to get the repo_name, since github is not sending it anymore, we can't use the gpc_get_string that ask to have the repo_name set.

Added the check of the api_key to be able to pass the IP check and import remotely with only the API key.

Fixed invalid XHTML bits ...
